You are about to express interest in signing up for a field trip hosted by the Experiential Learning department. All field trips for semester 2 for April-May are listed on the next page. Completing this survey does not secure a spot on the trip you request. If you secure a spot on your chosen field trip, your FBLT will reach out to confirm your spot. If you do not secure a spot, you will be placed on a waitlist and be notified that you are on a waitlist by the ExL department.

Please complete a separate survey for each student that you are requesting. You will be able to select multiple field trips for each student. By completing this survey, you are agreeing to the selected Field Trip date, time and EU allocation. Please review your choice carefully before submitting your request. 

Additionally, on page 4 of this survey is an important acknowlegement. Please read it carefully. Should your student get a confirmed spot on the requested field trip, the ExL department is unable to accommodate additional chaperones or guests on our field trips. Only the student(s) and chaperone listed in this survey are allowed to attend the requested field trip. We appreciate your acknowledgement of this matter.

Granite Mountain Charter School requests your permission to reproduce through printed, audio, visual, or electronic means activities in which uyour student has participated in his/her education program. Your authorization will enable us to use specially prepared materials to (1) train teachers, (2) create a yearbook, and/or (3) promote our school and education programs through the use of mass media, displays, brochures, websites, Yearbook, Newsletters etc. 

a. I, as a parent or guardian, of the aboved named student fully authorize and grant Granite Mountain Charter School and its authorized representatives, the right to print, photograph, record and edit as desired, the biological information, name, image, likeness, and.or voice of the above named student on audio, video, film, slide, or any other electronic and printed formats, currentlydeveloped, (known as "Recordings"), for the purposes stated or related to the above. 

b. I understand and agree that use of such Recordings will be without any compensation to the student or the student's parent or guardian. 

c. I understand and agree that Granite Mountain Charter School and/or its authorized representatives shall have the exclusive right, title , and interest, including copyright, in the Recordings. 

d. I understand and agree that the Granite Mountain Charter School and its authorized representatives shall have the unlimited right to use the Recordings for any purposes stated or related to the above. 

e. I hereby waive all rights, release and hold harless GMCS and its authorized representatives and shall neither sue nor bring andy proceedings agains any such parties for any claim, demand, actions, damages, costs, or expenses, including attorney's fees, whether now known or unknown, for defmation, invasion of right to privacy, publicity or personality, copyright ownership, compensation related to the use of the Recordings, or any similar matter, or based upon or relating to the use and exploitation of the Recordings. 

f. I agree that there is no time-limit on the valididty of this release nor is there any geographic specification of where these materials may be distribted or used.

Dear Parent/Guardian:

The mission of Granite Mountain Charter School is to develop the individual gifts of our students.  Our students will become proficient in Common Core State Standards and become critical thinkers, responsible citizens and innovative leaders prepared for academic and real-life achievement in the 21st Century. The mission will be accomplished in a personalized environment that fosters successful achievement through quality, personalized, standards-based education, which could include online coursework, offline textbook work, and unique hands-on and experiential learning experiences facilitated in partnership with students, parents, staff, and community.

Experiential Learning live and in person opportunities are designed around our school mission. During the school year, we will be recording our live virtual classes and events for reference and taking pictures of what is happening at our in person events. These videos and images may capture your child's participation, directly or indirectly. You may see your child’s image, name, and/or hear their voice in these recordings and/or images. These images and/or videos could potentially be shared in our school's yearbook, our weekly family newsletter, The Trailblazer, and shared at our school board meetings. A completed waiver is required to participate in both our live virtual classes and our in person opporunities being provided through the High School Activities Department and must be signed before the start of these classes. 

In addition to giving permission for recordings and images, we are asking that the policies within Granite Mountain’s Parent/Student Handbook as well as the following virtual code of conduct must be adhered to for students participating in our live virtual classes and in person events.

PARENTS, PLEASE NOTE: Extracurricular activities are not guaranteed and are at the discretion of the school and can be revoked at any time. The acceptance and inclusion of the student is at the discretion of School and subject to program standards and criteria. Student shall comply with all applicable codes of conduct and maintain high ethical and moral standards. Parents will be required to provide immediate supervision for their students during interactions with certain vendors to comply with state law. If immediate supervision is not possible when required, the specific vendor’s programs will not be available to the family. Vendors with "Parent Supervision Required" indicated on their vendor profile, as designated in our procurement system, have background checks that do not satisfy the requirement of AB 130 (Live scanned through the California Department of Justice and FBI). Vendors with "Parent Supervision Required" indicated on their vendor profile require parent/guardian supervision for the entire length of the service. The parent/guardian understands and willfully accepts to provide direct supervision of the student for the duration of services with the vendor.

ASSUMPTION OF RISK GENERALLY: By signature hereon, parent/guardian waives liability against and holds harmless the school and its board members, staff, volunteers, vendors, agents; the school district; and State of California; and further acknowledges that this voluntary activity and/or transportation to and from (as applicable) and/or access to and use of a dangerous product (such as sharp tools, chemistry kits, knives, paints, etc.) may expose the student to potential harm including injury or death. If student believes that an unsafe condition or circumstance exists with respect to the activity(ies), student will discontinue participation and immediately notify school staff. Student shall not further participate until the unsafe circumstance is remedied. Parent/guardian hereby agrees to remain responsible at all times for supervising the student(s).

I understand that the GMCS purchasing team will sign on my behalf. I agree that GMCS has my express permission to sign on my behalf any and all liability waivers for my child’s participation at said vendor. COVID-19 RISKS: I understand that it is my responsibility to ensure that the business providing the enrichment services identified above is following all local, state and federal health guidelines required in order for the business to provide said services. I understand that my child(ren)’s participation with in-person services during a health pandemic may result in my child contracting COVID-19, which may be spread to me and my family, which may result in severe illness, hospitalization and/or death. By signing hereon, parent/guardian waives liability against and holds harmless the school and its board members, staff, volunteers, vendors, agents, the school district, and the State of California relating to the contraction of COVID-19 to the student or his/her family. If the student/parent/guardian believes that an unsafe condition or circumstance exists with respect to the activity(ies), student will discontinue participation and immediately notify school staff. Student shall not further participate until the unsafe circumstance is remedied.

By signing below: (1) I am giving up substantial actual or potential rights in order to allow the student to voluntarily participate in this activity(s); (2) I have signed this agreement with full appreciation and understanding of the risks inherent in the activity(s) or use of the product; (3) I have no question regarding the intent of this agreement; (4) I, as parent or guardian, have the right to bind myself, the student and any other family member, representative, assign, heir, trustee or guardian to the terms of this agreement; and (6) I have explained this agreement to the student, who understands his/her/their obligations hereunder.
